A Case of Hypopigmented Mycosis Fungoides

Abstract
|
|
|
The hypopigmented lesion has been rarely described as a presenting form of mycosis fungoides. It occurs more commonly than typical forms of mycosis fungoides in a younger population and affects persons with dark skin almost exclusively.
Microscopic
features include lack of epidermal atrophy, moderate to extreme epidermotropism of infiltrating mononuclear cells and Pautrier's microabscess.
We report a case of hypopigmented mycosis fungoides in a 12-year-old boy. He complained of an increasing number of white patches on the trunk and extremities. These patches began appearing about 3 years earlier and were asymtomatic. The
laboratory
studies were negative or within the normal range. The histopathological finding showed a mild to moderate superficial perivascular infiltrate with epidermotropism of atypical lymphocytes and Pautrier's microabscess. On Fontana Masson staining,
melanin
decreased in the basal cell layer of the epidermis. After 7 months of PUVA treatments three times per week, he showed repigmentation with only focal areas of incomplete repigmentation. He showed no recurrence during a two year follow up period.
